If you’re 65 or older and enrolled in Medicare, you’re eligible for a yearly Medicare Annual Wellness Visits (AWV), but many clinics either don’t offer them or rush through the process. At FMP, we take the time to do it right.

Dr. Carol A. White is a board-certified geriatric nurse practitioner with more than 25 years of experience. She provides comprehensive 45-minute Annual Wellness Visits designed specifically for older adults who want to protect their health, preserve their independence, and age on their own terms.
This visit is not the same as a traditional physical. It’s focused on prevention, planning, and early detection, giving you the tools to stay well rather than just treat illness when it arises.

What to Expect During Your Medicare Annual Wellness Visits

This once-per-year appointment is covered by Medicare and includes:

  • A review of your health history and current medications
  • Screening for cognitive changes, depression, or safety risks
  • Assessment of your functional ability and risk for falls
  • Discussion of lifestyle, nutrition, and activity levels
  • Review of your providers and care coordination
  • Personalized health advice and a written prevention plan designed specifically for your needs

Prior to your appointment, you’ll be asked to complete a health questionnaire and provide documentation of your most recent preventive screenings – such as DEXA scans, vaccinations, colonoscopy, or cardiac stress testing – as well as a note from your last primary care visit.

You must have a designated primary care provider (PCP) – a physician, nurse practitioner, or physician assistant you see for routine care – to qualify for this service.

A summary of your visit will be shared with your PCP, and FMP will bill Medicare directly.

Important: There is a limited physical exam and it does not include labs, prescriptions, or new diagnoses. It is a planning visit, not a problem-focused medical visit.
