For new clients please schedule a discovery call before booking an appointment (excludes salt and sauna – go ahead and make those appointments!)
Have a list of questions ready and things you want to discuss.
No, you don’t! We carry premium quality supplements that are sold to patients and non-patients. Feel free to visit our Fullscript store for quality prescription grade supplements. The only brands that are not available for self-service per Fullscript policy are Apex Energetics and OrthoMolecular products.
You can expect future consultation every three months for the first year and then every six months after that based on your individualized needs. You will need to also get labs before these consultations. If you are being treated for thyroid or weight loss, those visits may be slightly more frequent depending on the treatment regimen.
We take credit card and Health Savings Account cards as form of payment on the website. In office, we will also take cash or cheque as well.
We are happy to print a superbill for services provided so you may submit for claims to your insurance company. Many of our services are not covered by insurance and we do not file with insurance in order to keep our costs lower.
No. Unfortunately the only way to determine if you are a candidate for specific therapies is by completing your blood work. There are certain medications and supplements that may be contraindicated based on the results of your blood work, and we need to know that before prescribing any treatment. Also, lab tests may pick up on a problem that needs further testing and referral to a specialist or your primary care provider.
Depending on your specific goals, you may not be eligible for treatment if you have normal labs and no symptoms, are taking any opioid medications( excluding bupropion/Naltrexone), or have specific cardiac, seizure or psychiatric conditions. You will not be eligible for treatment if you do not get your labs.
Functional medicine focuses on identifying and addressing the root cause of symptoms rather than simply managing them. Instead of offering a quick fix, we take a deeper look at your health history, labs, and lifestyle to develop a plan that supports long-term wellness.
We do not bill insurance for most services. By offering direct-pay pricing, we’re able to spend more time with each client, offer a wider range of treatments, and focus on care that truly works—not just what insurance will cover. However, we do bill Medicare for Geriatric Annual Wellness Exams if you qualify.
Yes. FMP is not a replacement for primary care. We work alongside your existing provider to offer specialized support for weight loss, hormone balance, gut health, and more.
Yes! We offer secure telehealth visits for clients anywhere in Indiana. Some services, like lab work or certain physical exams, may need to be done in person, but most consultations and follow-ups can be done virtually.
We use comprehensive lab testing to look at hormone levels, thyroid function, nutrient deficiencies, metabolic health, inflammation, and gut imbalances. Testing is based on your symptoms and needs, and results are used to guide your personalized care plan.
Just call our office or schedule online. We’ll guide you through the intake process and get you set up with your first consultation with Dr. White.
Bioidentical hormone therapy uses hormones that are chemically identical to those your body naturally produces. When properly prescribed and monitored, BHRT is considered safe and effective for relieving menopause symptoms and supporting healthy aging. We always base our recommendations on your lab results and health history.
Yes. We provide personalized care for both men and women, including hormone support, weight management, gut health, and performance optimization.
We offer a wide range of medications depending on your needs. Examples include (but are not limited to) Semaglutide and Liraglutide for weight loss, testosterone, HCG, B12, peptides, bioidentical hormones for hormone support, thyroid and gut medications, as well as vitamin support. All medications are prescribed based on lab testing and individual goals.
If you’re on Medicare and haven’t had a wellness exam this calendar year, you’re likely eligible. This 45-minute appointment focuses on prevention, not treatment, and is fully covered by Medicare. We’ll review your health history, discuss risk factors, and send a copy of your visit to your primary care provider.
You’ll receive a shipping confirmation email with tracking details as soon as your order has been filled and handed off to the carrier. This usually happens within 1–3 business days after FMP sends your prescription.
Each partner pharmacy uses a different carrier but most ship through FedEx. Some may arrive via USPS, or UPS.
You will need to message FMP regarding the medicine you are taking, why you need an early refill, and if there is a change in shipping address. You will be charged for the medication. The refill may be approved if you are low, or will run low due to travel or a delayed shipment, or if you need a higher dose.
Due to Federal Drug Enforcement Agency requirements, an adult signature is mandatory upon delivery of testosterone. If you worry you will not be home to get the delivery, you can either have it sent to your place of work, or download the FedEx Delivery Manager on your phone for free and you can redirect the package to a local Walgreens or to a Fedex location. Another option is to have the medication sent to another adult’s address to sign for it. The contents of your package are not disclosed on the outside. There will likely be a pharmacy address on the outside of the package.
